Media Services Request

    This form is to request any AV/Media Services equipment and services to be added to an already existing reservation on Ad Astra that was not added to the initial reservation.
  • All requests must be submitted with at least 48 hours notice. After 48 hours, you need to contact Media Services directly through the Help Desk: 281.649.3410.
  • Event must be accepted on Astra before this form is submitted. This number can be found on the Ad Astra confirmation email.
  • Please describe the equipment/service(s) that you would like to request to be added to your Ad Astra Reservation. Please note that the equipment that is requested on this form is not confirmed until you receive a confirmation email.
