Forms

• Change of Major Form (undergraduate) - Complete this form and bring it to the Success and Advising Center in Hinton Center room 101, for your major to be changed.
  
  
• Undergraduate Degree Plan Declaration -  Complete this form and meet with your advisor, and submit the form to the Registrar's Office in the UAC.
  
  
• Apply for Graduation - This form must be completed and submitted to the Registrar's office a semester before your intended graduation date.
  
  
• Transcripts - The Office of the Registrar provides official academic transcripts bearing the Houston Baptist University seal and the signature of the University Registrar. The University maintains an academic record for all students reflecting all courses taken at HBU, including undergraduate and graduate courses.  Transcripts can be ordered by mail, fax, or in person. There is no fee if five or fewer official transcripts are ordered per semester.

    Your grade point average (GPA) is calculated by dividing the total amount of grade points earned by the total amount of credit hours attempted. Your grade point average may range from 0.0 to a 4.0.

    This calculator has been designed to calculate the credits you need to improve your current Grade Point Average.   To use the calculator follow these steps:  
    
    1. Enter the number of graded credit hours you have completed (including "F" but NOT including incomplete
        (I) or withdrawals (W)), your current GPA, and your desired GPA.  
    
    2. Hit the submit button.

• Community Life and Worship (formerly Spriritual Life Points-SLP) - The CLW credits are a requirement for graduation. All students seeking their undergraduate degree from HBU must earn 80 total credits during their HBU attendance to qualify for graduation.
  
  
• CAPP - Online Degree Evaluation - CAPP is a web based report that helps track your progress against a selected degree program.  CAPP does not replace advising by the advisors.
