The following forms are for the convenience of the students in regards to the Office of the Registrar:

Request for Official Transcript
Re-order Diploma
Enrollment Certification Request Form
Add/Drop Form (Use this form for add/drop, closed courses, pass/fail designation & audit designation.)
Request for Early Grade Release (Complete this form if grades are required before the Official Release Date.)
Request for Off Campus Transfer Credit (Currently enrolled or continuing students use this form for prior approval of transfer credit.)
Request for Undergraduate/Graduate Course Notation (Transfer students use this form to request designated HBU course credit if general elective credit originally given.)
